Melawat klinik Audiology di JHC

Alhamdulillah, saat ini usai pembentangan BM dalam kelas cikgu Chow, amat berharap agar segala pengisian yang aku, kak husna, kak sarah, kak atiah dan shahira sampai kepada minda dan hati siapa yang mendengarnya.

Sambil menunggu aina ke klinik, aku mengambil kesempatan untuk menulis setelah hajat untuk makan tidak kesampaian. Mak cik cleaner yang ada melarangku untuk makan dalam surau, mungkin takut-takut aku akan mengotorkan surau ini barangkali, tidak mengapalah, aku akur lalu memgambil keputusan menulis ‘instead of’ makan.
Perut sudah memberi ‘signal’ ke otak, afferent neuron sudah berfungsi namun dibantutkan oleh cerebrum, tidak mengapalah. Kali ini aku ingin bercerita tentang kunjunganku ke klinik audiology, namun bukan sebagai audiologist tetapi ‘subjek’ pada ‘research’ akhir kakak izzati, 4th year, setelah diundang, aku amat berbesar hati untuk dijadikan subjek, mana tahu aku bisa beroleh sedikit sebanyak skop kerja sebagai bakal audiologist kelak. insyaAllah,(macam slanga indon pulak)

Mujurlah afiqah mahu menemaniku, bangunan JHC kupandang dengan pandangan yang kosong, mana x nya, kali pertama menjejakkan kaki ke sini. Nampaknya, tidaklah susah untuk mencari klinik audiology (pendengaran) kerana ianya hanya terletak di tingkat bawah. Kedatanganku disambut dengan cerianya oleh kak ijat, pada mulanya, aku diuji tahap pendengaranku, samada normal atau tidak. Pandanganku terpana pada deretan alat yang digunakan, Nampak sahaja ringkas dengan bentuknya yang tidak begitu menarik hati, namun harga bagi setiap alat mencecah puluhan ribu, maklumlah semuanya latan luar Negara. I wonder if there somebody could make it in Malaysia, x payah membazir kan?






Dengan bilik mempunyai dua pintu luar dan dalam, kalis bunyi dan pintu itu sendiri setebal 30 inci mugkin menyebabkan telingaku ‘berdengung’ kerana boleh dikatakan bilik ‘vacum’ jugalah. Kata kak ijat, kalu berlaku banjir ke kebakaran ke, belum tentu dapat kita dengar dari dalam.

Begitu pentingnya, setiap pemeriksaan audiology yang bebas daripada gangguan bunyi yang lain. Alhamdulillah, pendengaran aku normal dan tiada apa2 masalh. Aku diuji dengan bunyi2 yang ‘high frequency’ dan ‘low frequency’ atau ‘high pitch’ and ‘low pitch’…….untuk pengetahuan semua, secara normalnya, ‘hearing loss’ atau hilang pendengaran akan melibat kan bunyi ‘high frequency’ dahulu, seperti, huruf /s/, /sh/ berbanding ‘low frequency’ seperti huruf vowel.

Selepas, itu aku diminta untuk menjadi subjek pada eksperimen kak ijat, to determine the stimulation of outer hair cell which react contralaterally, menariknya, outer haircell yang terletak dalam cochlea sebelah kiri akan respod pada bunyi yang dihasilkan pada sebelah kanan(nasib baik ada buat assignment tentang ni) so, this mechanism is to protect our inner ear from high intensity sound, this is called olivocochlear mechanism. tengoklah betapa menakjubkan ciptaan Allah…..selain itu, bnyak lagi mechanism untuk melindungi bunyi yang kuat . Itu anugerah, Alhamdulillah, dengan nikmat mendengar ini, mari kita sama2 peliharanya supaya x dapat “hearing loss” pada usia yang muda .

Olivocochlear mechanism

First of all, when the loud sound travel to the outer ear, it is collected to be sent to the middle ear trough conductive pathway . Then, the signal is conducted trough 8th cranial nerve (auditory) which then travel to cochlear nucleus in the brain stem. After that, it is travel to superior olivary complex. This is the main checkpoint where from the brain stem, the signal is sent back to cochlea of the other side of ear trough Medial olivocochlea . If the sound is originated from left ear, it will be sent to the right cochlea. This is because, the MOC control the contralateral pathway. MOC is myelinated fibers originated in the medial part of superior olivary complex (SOC). This also shows the complex efferent system of descending fibers. Martin and Clark (2003) mentioned
These descending fibers correspond closely with the ascending fibers and connect the auditory cortex with lower centers and with the cochlea. One purpose of this descending system is to provide inhibitory feedback by elevating the treshold at lower station at the auditory tract (p. 316).
Martin and Clarks (2003) also adding in explaining auditory pathway, “descending efferent fibers from each side of the brain provide inhibition to both cochleas” (p.318).So, then the signal is sent to Outer Hair Cell (OHC) because MOC innervated OHC. As a result, the function of OHC in Organ of Corti will be inhibited or turn off then produce less intensity of sound.
In conclusion, besides other mechanism, these two mechanisms above involve in order to provide protection to the cochlea by exposure to the loud sound. Long term exposure by loud sound as stated before, will lead to Noise-Induced Hearing Loss. As stated by Jack Katz (2002), the early exposure to noise injury will usually produce a temporary treshold that depend on time which exposed to the loud sound while long term to large intensity of sound will induce permanent hearing loss. So, healthy practise of hearing should be improved in the industrial field where invention of the hering protection is emphasized to reduce the risks to the employers. This is the same with nonoccupational source of sound where people should be aware of preventing themselves from too much exposure to loud music by setting the limitation in using the gadjet such as headphone. So, people should appreciate hearing in their life which given by Allah to be used wisely in the journey to know Him and His creation well. (a part of the assignment)

akhirnya, aku dan afiqah dibawa untuk melawat seluruh klinik, blik yang terakhir yang kami lawati ialah bilik untup patient yang menggunakan hearing aid, hm lantas teringat pada beberapa orang kenalan yang juga menggunakan hearing aid. :)
